Keymapping partially resetting for no obvious reason
Knowing what and where you're trying to bind would be helpful, otherwise we're stabbing in the dark. (There are some buttons that seem to be totally reserved for City use and will remove manual binds after a while, like lbutton/rbutton.)

Please post the contents of your keybinds.txt file in your CoH directory.

I notice that sometimes that new keybinds (which I set using the GUI, like Power Tray 5 hotkey = R) reset when I zone.. In which case I set them again and they generally stick..
Something like this has been happening to me, but not only are my keybinds being jumbled (to activate powers I don't have), but it's also acting like my shift key is stuck so with every keypress I use powers in my second tray instead of the first.
Looking in my keybind menu it always displays that everything normal. I'd think it was partly something wrong with my keyboard if it wasn't also happening to my mouse buttons and if it didn't fix itself by zoning.

Actually I've been having odd problems too.
1. Keymappings don't stay put. Go to Menu->Options->Key Mapping. Change a few things around, but they might not stay put after I log. Really odd.
2. In keymappings, my \ (backslash) key no longer corresponds to \. If I try to set the Menu to \, I see ] displayed instead. It works: in game, it displays the menu when I press \, but if I go to key mappings it still shows ]. ] maps to [, and [ doesn't map to anything at all, it's ignored. Weird, weird.

This happens to me too, but with alt/ctrl instead, mostly when I alt-tab out of the game. I couldn't tell you a technical reason for WHY it happens, but tapping alt/ctrl fixes it.

Like you mentioned, pressing Alt again will fix it. That's because CoH will then witness you pressing Alt, and then releasing it, which will "unstick" the status of that key.
As for the OP's issue:
I've seen this happen before as well.
When we log into a character, information like keybinds and window status (open/close, size, color, position) is all loaded to our client. All that information is specific from character to character and is not contained in any files on our computer. The keybinds.txt file in your directory is only there to help us save and load our default keybinds. It does not necessarily reflect the keybind status of any particular character on the server. It's from the client that the keybinds then allow us to control our character, BUT when we change a bind, it saves on the client, but not necessarily on the character's file on the server. I haven't been able to figure out why it doesn't always save, but there may be a way to fix it... or at least it's worked for me in the past..
My theory is that the keybinds saved on the server become corrupted in some way, causing it to no longer accept certain changes, so the best thing to do is start from scratch!
Log into the affected character and make any appropriate changes to your binds that you see fit.
Then immediately (without zoning or logging off) do a /bindsavefile character.txt
That will save the binds the client is currently using to a file.
Go to the keymapping options menu and click on the red "Reset Keybinds" button.
That should erase any custom binds from your character's file on the server and replace it with a clean version the default binds as if you never made any changes the first day you logged in.
(Note: Loading a bind file does not replace the existing file, but simply rewrites only the keys listed in that file.)
After you have reset the character, log off immediately (and close client) and wait about 15-20 minutes to ensure the character is logged off and saved properly on the server.
Log back in and load your custom file: /bindloadfile character.txt
To hopefully ensure the binds save onto the server, log off (and close client) again and wait some period of time to ensure your new binds are saved.
Log back in and enjoy the character the way you designed it.
15-20 minutes may not even be necessary each time, but I'd rather not take any chances of the the character being stuck in limbo and forced offline when you log back in.
Should be in Program Files (x86)\City of Heroes
If it's not there, then you haven't saved your keybinds to the default file.
Set you keybinds the way you want it and use the command:
/bindsave
Or go to Menu > Options > Keybind tab and hit the save keybind button.
Then, without zoning. Exit to to the log in screen. Let the timer count you out. Then exit the game.
That should save it permanently for you.

Now that I've got a few new incarnate powers, I thought I'd reset some of my keymapping options
For some reason this was destined to fail.
The new mappings don't seem to want to stay put. Even if I save options they often go back to what they were (almost) after logging. Yes I'm clicking accept
The binds work for a while... then change back, or partially change back.
Most binds stay were they should be, but enough change to be infuriating
I recall one other character having this problem years ago, but I can't for the life of me remember the solution
Has anyone else seen this?
Any idea what's up?
