How does someone wear a cape before level 20?

Is that the same reason someone would have an aura at such levels?
Similar reason, yes. The Alpha and Omega auras are from GR Complete, and the Cyborg pack has a few as well.

also players can get shoulder capes and wear at level one vets
I have seen people with capes running around before level 20.
