It says your drivers are 6 months old. Did you get them from nvidia or from another site?
I am guessing not many people were running a $500 card on the test server, but it's not that you are crashing.
I think when I installed my video card I read that I should use the top slot unless running 2 in crossfire. Anyone think that could be a problem? I am not convinced it would be.
Was it visibly choppy in addition to reporting 15 fps?

Standard driver installation procedures apply. Download new drivers but don't install yet. Download and install your driver cleaner of choice but don't run it yet. Uninstall old drivers and software for them, reboot to safe mode canceling any new hardware wizards, run the driver cleaner you installed earlier, reboot to normal Windows mode canceling any New Hardware Wizards that come up. Once those are out of the way run the installer for the drivers you downloaded earlier.
If you want to check the driver install, you can run CoH Helper again and paste the results into Notepad and look at the Display Driver entry. It should have a date of 4/13/2010 and be the 197.45 driver version.
You could have stayed in bed and waited until you woke up to run it. Getting out of bed was your choice.

Ah, I actually did clean sweep again and installed those older drivers on a tip from another forumite on Nvidia. There was no change. Re-sweeping-updating again, anyway. The drivers are not the issue, me thinks.

As for using the first PCI-E slot, I think that is the recommended process, but I've seen people running video cards in other slots without this type of problem. Yes, one of them was a system running CoH. It let me show my nephew some of the improvements in his system since he'd just built it and knew what CoH looked like and ran like on his old computer.

Apparently they didn't do a Beta driver on the 197.44 for your OS. Those drivers supported OpenGL 3.3 and some people really liked them.
Since you are resweeping and updating again, post a fresh CoH Helper please.

EDIT: Out of curiosity, are you using a USB or PS/2 keyboard and mouse? Any other devices that might have a "listening" service similar to the ATI HotKey Poller?

Got rid of the worm.

Nvidia Control Panel is only throwing up number for one of the 295's GPUs. Instead of 1700ish memory, I'm only seeing 896. Harrumph. Bad card?
First off, let me apologize. I somehow read your first CoH Helper report as being Windows XP 64-bit. Don't ask me why, it doesn't make sense to me either.
That being said, I see a few things that could be causing some problems. Let's tackle them one at a time and see if things improve and if not, move to the next.
First off, AVG. I love it, but sometimes I hate it especially lately.
Go into the Anti-Virus portion and set the CoH installation directory as being excluded. Do the same for the Firewall portion and set the cohupdater.exe and cityofheroes.exe to be allowed. Sometimes updates to the updater make the firewall think it's a different program so it has to be removed and readded. These AVG components are also "listening" in the background so that could be causing part of it as well.
If that doesn't help, try disabling the Steam Client Service. The next one I'd try disabling is the Andrea ADI Filters Service. This is a service that aids in the reduction of background noise when you use a microphone. It does nothing else. Again, it may be using a "listening" type agent.
Another thing, several people were trying out the 197.44 drivers that have OpenGL 3.3 and OpenGL 4.0 support. You can download them at the bottom of this page if you want to try them. Some said they worked better for them than the 197.45 when they were released. YMMV
EDIT: To clarify, one of the things I hate about the new AVG is that you no longer have an Exit or Quit AVG from the icon in the taskbar. I used to use that to quickly get around software that didn't want to install right with the AV active, but now I have to open it up fully, go into the Resident Shield and turn it off, then remember to turn it back on since it doesn't come back with a reboot.

Okay, that just eliminated one of the troubleshooting steps.
I'd suggest contacting nVidia and the card manufacturer about those readings then. You may indeed have a defective card.
I'll not ask that you remove the motherboard from the case so that you can fit the card in the slot to see if that's the problem. (The technical support people for the card may insist on it however to make sure it isn't a slot problem.)

Okay, that just eliminated one of the troubleshooting steps.
I'd suggest contacting nVidia and the card manufacturer about those readings then. You may indeed have a defective card. I'll not ask that you remove the motherboard from the case so that you can fit the card in the slot to see if that's the problem. Edit - What I'll do tomorrow is open up the card itself and see if the small internal SLI bridge is loose or something. Maybe it or something else will stand out.

Hm, I thought of that. The board has 6 PCI-E slots, and I tried it in two of the dedicated x16 slots (though still not the first). It would be a royal, royal, rooooooooooyal pain in my tuckus to move the HD's down into the secondary bay but.... possible.

So, good news and more bad news.
Good news: Switched out the HDD bays on my Antec 900 so now the full bay is on the bottom and the 295 was able to fit quite nicely into the primary PCI-E slot.
Bad news-ish: No change. Luckily, my friend got a deal on a bulk order of Nvidia 480's and he's giving me one at 75% of market. Yay.
Worst news: While switching my HDD bays, one of the wires came out of the fan mount and nearly shorted out my motherboard. Can't get it back in >
My Antec 900 now only has one front fan. Grumblegrumblegrumble.
Glad you were able to eliminate it being the slot. Sorry it turns out to be the card. I'd still call the card manufacturer as it may be a fault they have become aware of and may replace it. This is one of the reason's I prefer buying XFX cards. Their warranty would have replaced the card anyway.

Make sure your using tripple buffering if your using vsync - try playing without vsync also if your using vsync to see if problem goes away.
Make sure your not forcing SLI mode on (GTX295 being an SLI setup an all) as forced SLI + City of Heroes on default settings will result in slowdown and or flickering graphics.
Use GPU-z to make sure the PCI-e slot your 295 is in, is infact running at atleast PCI-e 1.x 16x speed or PCI-e 2.0 8x - anything lower will cause slowdown with the GTX295 as it has an internal PCI-e splitter.
SLI mode should be enabled on the 295 - you can enable it in the nVidia control panel - this won't make any difference to COH performance unless you play with unsupported tweaks to try and brute force SLI, but it will make sure your card is functioning properly.
For reference - I'm playing with an overclocked GTX260 SLI setup that is essentially similiar performance to the GTX295 and at 1680x1050 I get around ~45fps in outdoor scenes with long view distance, ~70fps in scenes with medium view distance and ~180fps while doing missions solo inside using all settings maxed with 4x AA and Ambient Occlusion set to performance/medium bilateral settings. If I'm missioning with a decent sized team I turn AO down another notch and the ultra settings down 1 notch (except leaving shadows on shadow maps) so I get around 55fps constantly in missions for smoother team play.

See, I'm getting worse performance. On what is essentially two GTX 260's slammed together. The second GPU is the culprit. Just wish I hadn't snapped my lower front mount fan to figure it out. > : (
I just can't seem to get a handle on this. System is freshly built by my own hand, everything is working perfect.

And then I play CoX, and I'm getting... 15FPS in Cap? I made sure to turn Occlusion off while running FSAA, but... well, I don't know. I just feel like I'm not getting the proper performance out of what I have.
And what do I have? This (on laptop, no DXDiag for the desktop tonight):
Motherboard: ASUS P6T7 WS Supercomputer "Ultimate Gamer"
CPU: i7 960 3.2 (not OC'd, waiting on V8 Cooler)
RAM: 12GB DDR3 standard Corsair (nothing fancy, just your basic stuff)
Primary HD: Intel X25M 160GB SSD (<--- CoX is on this, however)
Data HD: 1TB Seagate 7200RPM
PSU: InWin 1200w Commando <---- I know, right?
GPU: Nvidia 295 with a small amount of overclocking. Drivers are up to date and I did a full SafeMode/Driver Sweeper just to be sure. Rolling at 1920 resolution.
The only thing I can think of is that the 295 is not in the first PCI-E slot, but it is in a x16 slot. Something else that caught my attention was, using GPU-Z, only one GPU of the 295 (which is two smooshed together) was getting hot. The first hovers between 55-65C while the other just seems to be idle at 47ish.
Any thoughts from anyone? The 295 should be killing CoX with a 1200w PSU.
What is happening???? ;_;
Also, using GPU-Z, I notice that SLI Mode is disabled. Is the 295 suppose to be recognized as an SLI card?
