Dual Blades sound issues?

I played my DB/SR scrapper for the first time since I-16 last night and it seemed like I was getting the "whiff"ing miss sounds even when I was actually hitting. I was playing with the difficulty slider so things were happening crazy fast and I might be incorrect, and with all the extra tick dots and AOE orange numbers I didn't notice the "miss", but I'm sure I noticed the sounds being off. The whiff sound, but then my next power would get the orange circle so I'd just continue my chain and everything worked out. I run focused accuracy and was popping yellow insps as fast as they dropped, so I'm sure I wasn't missing much at all, if any. Maybe it was on the AoE or cone powers, I'd hit a couple guys but miss one and that would prompt the whiff sound? I dunno, I'll play with it some more tonight, just curious if anyone else noticed an issue or was it just me.

As a note, I only run the sweep and AV combos, no BF or other DB attacks. I'm running a level 47 DB/SR scrapper, most all of my attacks are fully IO slotted.
