*I bought Going Rogue via Steam when it came out; so I've been having no problem playing COH through it since then.
Two things jump out at me right off the bat.
Windows has problems with some functions when Free Space gets below 20%-25%. It's not looking at the amount of Free Space itself, just the percentage of the Free Space.
Several of the routines within windows seem to be using the same coding they used back in the days when a 20 Mb Hard Drive was considered big.
The second thing is all of the entries showing with Unknown in the CoH Helper report. That doesn't let us see what the game settings are. If you can get the Settings button to work on the launch screen go in and turn Ambient Occlusion completely off.
Did you right-click CoH Helper and select Run as Administrator? If not, please run it again doing it that way. If you did run it that way there is something keeping CoH Helper from reading the registry and that could be causing some of the problems.
A HiJackThis report would be very helpful to see what else might be running on your computer that could be interfering. -

The design and plans had to be changed because they found that people were either making tank-mages or were severely gimping their characters.
When they made those changes they implemented the AT system instead with specific primary power sets and secondary power sets to choose from and then pool powers that any AT could choose from.
Origins were kept in the game but had little affect other than your starting contacts in the game, the titles you could choose at level 15 or 25 and what DO and SO enhancements you could slot.
Later they added an origin-based temporary power that you would lose once you reached Level 10. This was later changed to be a power that you kept at all levels.
They made some additional changes to the game later that had a minor emphasis on origins (which Origins got a better boost from the vet powers like Nemesis Staff or Blackwand), the text in the Origins of Power arc and what you take from Trapdoor in Ramiel's arc. -

Since OpenGL 1.4 is the latest version that the drivers for that chipset supports, it's highly likely that what the OpenGL check is looking for is something from a newer version of OpenGL. It's also possible that it's due to the way the drivers were written. Intel has always had poor OpenGL support in their drivers. If they didn't code their drivers to handle whatever call the game is making during the OpenGL check, it's going to fail.
The Windows built-in drivers don't have OpenGL support. It's possible that the Lenovo drivers don't have full OpenGL support. Intel has always had poor OpenGL support in their drivers (which makes it harder for manufacturers to provide drivers with good OpenGL support). -

One even had a screenshot.
Since that is a Visual FX thing it might be Honey Badger's area.
Or you could always just PM Arbiter Warrant pointing him to this thread. -
I'd recommend turning Ambient Occlusion completely OFF.
That Babylon toolbar can be a bugger to remove at times. I generally go into the browser and disable the Add-On's there and then remove it from Programs and Features.
That generally seems to get rid of it easily.
And I hate that it installs even if you tell it not to. -

They were trying to add them as part of the normal leveling up process for all players. -

The Game started as City of Heroes with a required sub of upto $15/mo.
Issue 6 (I'm pretty sure) introduced the Villians side of the game, but you had to purchase the upgrade/addon content. It was probably something like $20 at the time, but it was a one time thing.
A few other content addons worked like that until Going Rogue (which you still had to purchase) at which point all previous content was unlocked, but again, still the same sub.

There were no other addons that worked like that. There were some booster packs that were released for purchase but there was no content in them, just costumes, auras, emotes and at times a power.
When Going Rogue was released it was a full expansion, meaning you had to own the (by then combined) City of Heroes/City of Villains game and have an active subscription. With the change to the City of Heroes: Freedom model, they combined Going Rogue with the City of Heroes/City of Villains game for subscribers. Free/Premium players can purchase Going Rogue if they can find a copy or if they already had it on their account before Freedom it would still be available to them. Otherwise, you can only purchase a la carte items like the Alignment System or the ability to create a Praetorian.
All other issues were Free until Going Rogue came out. All issues since Going Rogue have been Free.
EDIT: According to the NCSoft Press Release from November 1, 2005 the price for City of Villains was $49.99 for the standard edition and $69.99 for the Collector's Edition.
EDIT 2: The Going Rogue Complete Collection was priced at $39.99 at release. The purchase from the NCSoft store of just the Going Rogue expansion was $29.99 and they also had the Going Rogue Item Pack avaialble for $10 dollars. -
