Recently I made a spreadsheet to figure out the accuracy needed. I just uploaded it to Google Docs. It's not pretty, though
Accuracy Spreadsheet
It shows the accuracy needed to reach 95% against enemy levels from -4 to +7, and from 75% base ToHit up to 120% base ToHit. Bolded lines at 75% (basic ToHit) and 81% (Karma +Acc IO).
If anyone notices any mistakes with it, let me know. -

It's working fine here.
Have you maybe picked up the regular Blackwand power and are using it by mistake? Are you using a bind to activate the power, which might be activating the regular Blackwand? -
I've noticed the same thing from time to time, though I haven't investigated it further yet.

This isn't happening everyone else, so you might want to check to see if you drivers are up to date, run a virus and malware check, etc. It seems more likely that the problem is on your end rather than the game's end. -
I bought 4 shield wall procs a few days ago for a total of 1 billion. Converters have crashed the market on these. See the various threads on the subject in this forum.

Simply put, you gain Reward Tokens by spending money. First purchase of any amount (min $5), every 1200 points purchased on the Paragon Market, or each month VIP gains you 1 token.

Use inspirations constantly. Try to keep 1-2 lucks going throughout the battle, but no more than 4 at one time.
Get a Shivan from Bloody Bay.
Unless you're normally fighting +2 or greater, replace one of those accuracy SOs in your pets for a damage. While you're running Tactics, your pets will still be capped at their chance to hit with just one and the damage will help kill faster.
While it can't help you now, on your next respec, remove the Defense and End from Hover. The defense in Hover is so low that enhancing it isn't worth it. Same with it's end cost. Also, remove the fly enhancement from Fly. The speed of Fly is hard capped without enhancement. -
I use base port to Striga and then Ouro portal out when done. There are a lot of bosses in that area and I concentrate specifically on them. I'm usually in and out in 2-3 minutes.
