Tokens from subscription time come at the END of the paid time, just like the old Vet badges did (for the old-timers).
You should have:
-One token (for all accounts)
-One token (for your first point purchase)
-One token (for 1200 points purchased)
-One token (for your first month as a VIP player, awarded on Jan 16th per your OP)
Your next token should be on Feb 16th, at the end of your first paid month.
Everything is in order. -
You CAN'T fill in the Tier 9 rewards, all are repeatable consumables. T9 and T9 VIP are both available to you immediately.

I was under the impression if I went back to pay to play, $14.99 per month or so all of my toons would be unlocked since I was paying for them when they were all created. Is this incorrect?

There are a few pieces (like certain jackets) that have three player-selected colors, but the third color is a blend of Color 1 and Color 2, not an actual third color. -

The original fix was to make it so that the spawn size never dropped below the minimum for the Task Force. So for example, on a Manticore Task Force, the missions would never spawn for less than 7. However, this brought out the griefers: Groups of 3-4 players would join a Task Force, and then midway through, all quit, forcing the team to struggle through oversized spawns or disband and start over.
The final change was to make it as I listed above - spawns are sized for the actual team, but it counts everyone, even the people who are logged out. Since we can't kick the people who are logged out, some griefing is still possible, but it's less likely. -

Free and Premium players can join all TFs and Trials (except Apex, Tin Mage and Incarnate Trials, which have Incarnate requirements that non-VIP accounts can't meet), which means that if you have a good enough computer or enough computers to run multiple accounts, you CAN start TFs "solo" by creating a batch of free accounts and leveling characters to the minimum level for the TFs you want to do. -

I used to play a couple Facebook games that used this method of eraning free game money.

You're right, they did fix it a couple years ago. However, the fix never made it Live; it just showed up (undocumented) on the Test server, and vanished just as mysteriously. I can only assume that whatever they did to fix it broke something else, and that whatever it was was considered worse than the problem that they were trying to fix.

Step 1 - Take all the powers from Super Reflexes (Elude is optional)
Step 2 - Put 3 level 50 Defense Buff common IOs in each of the Defense toggles and auto powers. Remember to add some endurance reduction to the toggles.
Step 3 - Take Combat Jumping from the Leaping pool. Put a level 50 Defense Buff IO in it.
Step 4 - Take Boxing or Kick (your choice) from the Fighting pool. Most people never use this power, they just use it to unlock the other powers. Take Tough and Weave. Put three level 50 Defense Buff commons in Weave (and some End Reduction, of course). Put a Steadfast Protection: resist/+3% Defense in tough, in addition to Resistance Buff and Endurance Reduction to taste (I just use the Steadfast, and never even turn on Tough).
Step 5 - Put all 6 pieces of Gaussian's Synchronized Fire Control in Build Up (or Follow Up, Blinding Feint, Soul Drain, or whatever Build Up equivalent your set has) or in Tactics (from the Leadership pool) or in Focused Accuracy (from Body Mastery, Weapons Mastery or Mace Mastery in the Epic pools).
If you plan on exemplaring a lot, get the seven IOs I listed above at the lowest levels you can, so you don't lose the bonuses while exemplared.
With this slotting, you'll have about 45% defense to Melee, Ranged and AoE, and the cost will be relatively low. Remember to use your Alignment and Reward Merits to pick up a few of these IOs if any of the prices seem too high on the market. 45% is a significant number (generally called the "Soft Cap"), because at this point, larger bonuses don't reduce an enemy's chance to hit you any more; it only serves as a buffer against defense debuffs (the above build caps your Defense Debuff Resistance, so that's of little impact to you) and/or To Hit buffs on the enemy (which suck. You'll need to learn who the buffers are and take them out first).
Of course, you can drop the extra powers and make up the difference in set bonuses, but I find that this combination is the quickest and cheapest way to the soft cap; if you try it and don't like it, the loss is pretty minimal. -

On Freedom or Virtue I've had months without seeing one. But on Exalted from day one I get 1-4 a week easy; and a few weeks even more. Had 3 ITF's were I got 2 purples.
Am I or is this server lucky? Is it because its VIP or a smaller population?
I suppose it's possible that drop rates are higher on Exalted, but I highly doubt it. The devs have always said that they prefer to keep the same code on all servers. -

This toon doesn't have SS - yet. I plan to respec that in soon. Just wondering about this variation. -
If I have a Stealth Proc in a quick recharging click power (for example, Celerity in Speed Boost), and I use the power multiple times, will the proc stack?

All of the Praetorians from the various incarnate trials are supposed to count. I understand that the MoM trial is (was?) bugged and wasn't giving credit, but if that hasn't been fixed, it is in the current Test and/or Beta build.
I imagine that once the Praetorian storyline fully unfolds, we'll be able to get Dimensional Warder without ever doing a single mission from Maria or Tina.
