Anyway, long story short, I'm fairly sure that's what happened to the OP. He was closest to a door that sent his character into a room with no exit.
This door trick is also key to getting under the map in some zones. Simply relog in front of a buried door.
On a side note.. You coming in my door all the time is bad for my business. lol:

The caves are also still blocky and I hope that gets corrected , too. This is such a beautiful game and bad textures really make it look 'cheap'. Probabbly not high on the list of priorities, but sure hope it will be resolved sometime soon. -

..and no, it's not just because I had something bound to that key previously, but I actually rebind it on-the-fly several times a day:
/bind DIVIDE "beginchat /bind TILDE targetcustomnext enemy alive "
I simply press / and type the name of the enemy I want to search for (usually a boss, or General, or Cyst), then spam the TILDE to search for my intended target.
Still works flawlessly for me. Not sure anybody has figured out what makes this stop working for some people. -
I realize the crates each have names, but it seems to be bugged once in a while. Our 8th crate on our last run was green and said Thermite, but the person who clicked it was awarded the Particle cannon anyway. That's the only reason I say it's not 100% accurate, but pretty close.

In my experience, you can usually tell what's inside the crates:
Orange tinted crates are usually Particle Cannon.
Green tinted crates are usually Thermite Cannon.
This is not 100% though, but if you are looking for a certain weapon, it's worth a shot. Out of the last 4 runs I did, only 1 person's crate was wrong.
I recommend that the Scrappers and Tanks take the Particles when they can because they will be less likely to die from the ambushes. The idea is to keep the 4 people with Particle Cannons focused on the head the whole time the generators are down. If they ignore everything else at that time, and can withstand the ambushes, the Head can be defeated in only 2 waves. The other tactic is just the opposite.. let the tanks and scrappers keep agro of the ambushes while the squishies use the Particles on the head.. but I've found my first suggestion to work more often. Bringing a couple Purples along never hurts either. -

Thankfully, the winner of the final match did get the badge every time, but figuring out who advances and who doesn't is messed up.
(We were using 8 entries. Because there was no elimination to the top 8, this may be what is causing the unusual behavior.)
Round 2 is the winners: 1v1, 1v1 = 4 people
Round 3 is the winners again: 1v1 = 2 people
Winner of round 3 = Badge.
If you were quitting the match too soon or not staying in the match long enough to count as a win, then it might screw it up also. -
I dinged Immortal when I logged in, but I admit I was already farmed up to about 90% of it's previous requrement before then. lol

Are you using the same resolution as your desktop? Having the game set at a different resolution on Fullscreen, then trying to switch back and forth may have odd results.
What if you just hit the Windows key? Do you end up seeing the taskbar and start menu pop up and the game still running behind it? -
If you are going strictly for badges, you don't even have to defeat anybody. The problem is only 1 person can get the badge at a time, so you would need to do it 8 times for each of the members of the team.
Before you start, you all agree who is getting the badge (we'll call him Badger).
Once the round begins (not when you zone, but after the counter runs down and buzzer goes off), everybody except Badger will quit the match. The people who ended up quitting first should end up being the loser, and the other person would advance to the next round of only the 4 winners (including Badger cause he never quit th match and automatically won). Then you repeat it again so you'll have 2 winners, one of them being Badger again. Lastly, the Final round will begin with only the Badger and the other winner. You do the same thing again. The other person quits and the Badger should stay in the mission until the badge is awarded.
Repeat the entire process again with a different Badger.
(In my experience, the match gets bugged if people quit the match before it actually begins. Sometimes the other person isn't fully zoned, or because the match never went through, the winner isn't always who you want it to be, or there might also end up being "ghost matches" where only 1 person zones into the nxt round and automatically wins.. or get's stuck there. This doesn't ALWAYS happen, but to be on the safe side, just wait til the buzzer goes off and you shouldn't have to worry.)
Also, nobody should attempt to get the next match ready until the first one is completed and Badger has the badge. Even though you lost, you're technically still part of the event, and joining a new event might mess up the first one. It's perfectly fine to open up your current event to check the status of winners though. -

If you do that, you're still stuck with having to defeat the Essence because they will most likely not be in Line of Sight of Rom when he dies.

The Nictus wont follow, and you can even try to taunt them and they wont leave that area. Just make sure you keep a LoS on the Nictus so he can use them to rez. -
When Rom dies, he uses the essence to Rez himself (if he has Line of Sight of one). When he does this, the Essence dies. Simply kill Rom multiple times until they are all gone.

Although, I have had the Longbow Warden (healer) since the PvP Mish Glitch of '05; I have never farmed healing because I just could never get anybody together to work an Arena Farm. Also most of that healing was off my pets since I solo for the most part.
Thankfully, the numbers were 'normalized' to a rational level.
