You can keep failing, or you can keep succeeding. The problem is, you have to do them multiple times just to unlock the slots. A much better design would be to have a success award a huge chunk of iXP, IMO.

No, not really. To my understanding, giving everything +acc would raise the to-hit "floor," and make defense buffs less useful. Whereas giving everything +to-hit just encourages those softcapped people to invite a bubbler or eat some purples.
Not stupid DE quartz-level to-hit of course, that's just stupid. -

Softcapping doesn't really help either. Just FYI. I brought my soft-capped DM/SD on Lambda and managed to die just fine.
I also managed to not die quite a bit, so there is that. -
Your point? Yeah, I didn't take Crippling Axe Kick until it got the alternate animation either. Some things are just too fugly to use, and too time-consuming to get just for the level shift and never use.

I failed three Lambda trials and walked in on the tail end of a "we're going to fail anyway so let's eke out as much iXP as possible in the time we have left" on which I wasn't placed on the team, and unlocked my Interface.
Now to fail some BAFs to unlock Judgement. -

Right, because this is a comic book universe where the only sane, rational response to any sign of aggression from a dimension that has been nothing but friendly to you up until then is a Daffy Duck-esque "of course you realize, this means war." Other dimensions. They are all just amorphous entities where everyone is the same. Which is exactly how the Portal Corp missions present most of them, so who can blame the Rikti or Praetorians for thinking our dimension is just like the one where the secret exclusive hand-picked girls-only assassin cult makes up the entire population?

Groups that would make total sense are Crey, Arachnos, and much as I hate to say it lest someone up there get any ideas, Nemesis. -
The Nemesis/superdyne connection is at least a logical conclusion to draw, if it's not explicitly in the game in some random one-off mission in a level bracket that has enough arcs that nobody ever does random one-off missions anymore.
When the story builds off what came before, that's evolution. When it changes what came before, that's a retcon. See the difference? -
Who knows? Does it matter? Superdyne and Dark Watcher aren't the only means to access other dimensions. The CoT have tried it, so it logically follows that there must be magical means. Dark Watcher had the help of the Midnighters to trap Rularuu in the Shadow Shard, and if magic can seal a dimensional barrier, it should also be able to open one. Furthermore, the Praetorians have accessed other dimensions than our own, since Battle Maiden and Infernal are not native to there. And where are all the magic users in Praetoria anyway?

You can slap a BotZ in your travel power, so you don't have to use up a slot in one of your toggles for it. The Karma and Steadfast -KB IOs don't actually enhance the resistance or defense of the power they're in. The BotZ doesn't enhance travel speed either, but if you have Super Jump or Super Speed the base speed should be good enough. BotZ can't go in Sprint.
Each of these enhancements gives 4 points of KB protection, which should be good enough for most things at lower levels, but post-40 you'll probably want two. With two you should be good against pretty much everything except Mary, Fake Nemesisises, RWZ pylons, some AVs/EBs and probably some Longbow bosses. -

1: NPC who shows up everywhere even though nobody cares about him? Check.
2: Organization that has been around for ages but we've never heard as much as a whisper of before? Check.
3: Plotline that makes sense as is (Praetoria fired first) and doesn't need to be retconned? Check.
4: A retcon that would be a hamhanded cludge at best and doesn't in any way build on pre-established game lore? Check.

The ones in the Shadow Shard bug me extra hard because they would have been a perfect opportunity to introduce some more background lore on the Shard. Instead, they use them to try to convince you the Shard is a creepy place. I wasn't creeped. Now those twisted trees in the Storm palace, the Storm Palace itself, the rivers of blood (that I am not stupid enough to drink, thank you very much whoever wrote that badge text), Ruladak's cave, giant chomping eyeballs....those are creepy, because they are so alien. The badge text is just...there. It tries too hard, and fails utterly as far as I'm concerned. -

No, the game never uses the word "Nazi." They're different enough from actual historical Nazis so you can have all the fun of beating them up, but none of the controversy of having real Nazis in the game. They are also, as far as I know, never presented as having a racist agenda, just a world domination one. But do you really need it spelled out for you? A German villain group that formed during WWII and fought for the Axis...what else are they? -
