Hmm, perhaps you received it and don't know it -- are you looking in the "Gladiator" section of your badges for it? (It's not in the "PVP" section.)

No, we won't know until a dev comments, or we see something during (open) testing.
As it stands now, no, Primal Earth toons wouldn't be able to get the badge since it exists in the tutorial for Praetoria... -
Magellen, I recommend you drop in on the PvP forums and ask over there. I'm sure you'll get some great advice on what build(s) you might look into.

Umm, yes, but only for that "session". Try it and see what I mean -- the warning will be back the next time you interface with your salvage (even during same log-on session).

Since he IS a GM, yes, I believe that the usual GM badge rules apply, which is that your team has to do 10% of the damage to get badge credit toward the GM.
From the wiki:
When defeating this monster you would have to have done at least 10% of the damage to it to receive an award. This would mean 10% of total damage done to the monster, not its total Hit Points. This includes any damage done by opposing NPC mobs that the Giant Monster may have been attacked by before you defeated it.

A note on level, in my experience: I've been purchasing and crafting LotG +Recharge IOs, and I either cash my HV Merits in for a level 25 (lowest level for LotG) or a level 50, because as a general rule people seem to more highly value either an extremely low-level IO (such as for a lowbie, or for exemping purposes), or a max-level one (50 for LotG, 40 for Miracle, etc.). -

Usually you have to wait for a new version that is compatible with the current Live build.

Also, many peoples' problems can be solved by right-clicking and selecting "Run as administrator". If nothing else, give that a try.

Yes indeed, they were "bound on pickup" during the I18 Beta preview we had. Who knows whether they will be when I19 happens, but I'd be willing to bet that won't change.
