I have to agree with Bill--while the online store could make some money selling the codes, I think I'd prefer to earn the costumes permanently in-game through some manner of tertiary non-game-influencing system like basic badges/crafting.

Yes, at the conclusion of a successful mission the standard rewards table pops up. Ergo, different members on the team can select different rewards after the same mission. -

Halloween salvage is also "special" and does not affect your regular salvage capacity. Just ignore them until next summer, and sell them at a profit.

I've visited her multiple times, and she's always been there. The wiki page reports that some players have difficulty finding her. I don't doubt that it is a bug, but I would try again:
